Please note that evenings after 6:00 PM and Saturdays we may close early if there are no remaining appointments or for staff training.
If you need to pick up forms or other non-appointment reasons. we suggest you call by 4:00 PM Mon-Fri or by 10:00 AM on Saturday to be sure someone will be here.

Our advice nurse is not available on weekends, however we do have an on-call physician available for emergencies.

Specialist Referrals
Please note that referrals require up to 72 hours to process. When requesting a referral, please include your name, insurance company, ID number, specialist name and specialty. We will notify you when your referral is prepared and can fax or mail it to you, or you may pick up the referral. Same day referrals are limited to medical emergencies. Referral requests may also be submitted via the Patient Portal.
