What Does PCMH Mean For Our Patients?
MedPeds is proud that we are recognized as a Level 3 Patient Centered Medical Home or PCMH.
What is a Patient Centered Medical Home?
As a PCMH we are dedicated to not only treating patients who call our office for an appointment, but to reach out proactively as needed to ensure patients receive recommended preventative care, are followed-up after hospital and or emergency room admissions, have access to our online patient portal, appointment reminders, etc…
We feel that this approach results in healthier patients, fewer visits to hospitals and helps contain the skyrocketing cost of care.
We are one of only 53 practices in the state of Maryland selected to participate in a state-sponsored PCMH pilot. Patients may choose to opt out of the pilot, but opting out does not affect the level of care you will receive from our office. If you opt out the only difference will be that you will no longer be included in statistical reports generated to evaluate the program.
Rest assured that no personal information about participants in the PCMH program will be shared with the state, regardless of whether you opt out or not.
We hope that you will not opt out of this important program, but if you prefer to opt out, you will need to complete the opt out form available on our website and return it to us.
